EDUCATION: Master's degree (MA/MS) in nursing as a family nurse practitioner from an accredited four-year college or university required.

EXPERIENCE: One to two years related experience and / or training in clinical specialty area required.

CERTIFICATE, LICENSES, REGISTRATIONS:
" Current registered nurse license and advanced license from the State of Nebraska.

" National certification in areas of specialty.

" CPR certification.

" ACLS certification if needed for specialty license.

JOB SUMMARY: Responsible for providing primary care services, including assessing and managing patients in the clinic following established standards and practices.

PRIMARY JOB DUTIES:
PRIVILEGES: The applicant must submit a privilege request form in ! conjunction with his / her request for appointment as an Allie! d Health Profession (AHP). Requests should include only those privileges that correspond with clinical privileges held by the collaborating physician(s). The collaborating physician(s) must confirm a willingness to supervise the applicant in performance of the privileges for which the applicant seeks appointment, identifying any exceptions. The employee must demonstrate, to the satisfaction of the Committee, appropriate training to perform privileges, unless excepted. All privileges must fall within the terms of the integrated practice agreement between the applicant and the collaborating physician(s) and within protocols describing when, how and under what circumstances and supervision the privileges will be performed. The request for privileges shall include the age category for which privileges are requested (All Ages; Children 3-12; Adolescents 13-17; Adults 18-64; Geriatric 65+). The privileges authorized for qualified, registered APRNs, as relevant to the area of spec! ialty, include:

a. Core Privileges. Core privileges include the initial and ongoing assessment of the patient's medical, physical and psychosocial status including:

i. Obtain a medical history, under physician supervision

ii. Perform a physical examination, under physician supervision

iii. Conduct preventative screening procedures

iv. Identify medical and health risks and needs

v. Formulate the appropriate differential diagnosis based on the history, physical examination and clinical findings

vi. Identify needs of the individual, family or community as a result of the evaluation of collected data

vii. Order appropriate diagnostic tests

viii. Conduct and interpret identified diagnostic tests based on demonstrated training and experience

ix. Identify and prescribe appropriate nonpharmacological interventions

x. Evaluate and initiate medications including controlled substances ac! cording to Nebraska statutes and within the framework established by co! llaborating physician

xi. Develop and provide relevant patient education

xii. Make appropriate referrals to other health professionals and community agencies

xiii. Determine the effectiveness of the plan of care through documentation of client care outcomes

xiv. Reassess and modify the plan as necessary to achieve medical and health goals

xv. Participate in quality assurance review on a periodic basis, including systematic review of records and treatment plans

Core privileges include the performance of any interventional techniques that are a natural extension of a designated procedure set out above.

b. Specialty Privileges

i. APRN Cardiology Privileges

(1) Observation and monitoring of stress tests. Must follow established criteria and protocols. Physician supervision shall be available at all times, either on-site or by telephone, if consultation is needed for any reason.

(2) Initial! cardiology consultations on Hospital patients (as defined in the Job Description for Cardiology Advanced Practice Registered Nurse by the sponsoring physician(s).

(3) Assist with insertion of Swan-Ganz catheters

(4) Assist with insertion of temporary pacemakers

ii APRN OB/GYN Privileges

(1) OB/GYN oriented history and physicals

(2) Normal pre-natal and post-natal care

(3) Normal pre-operative and post-operative care

(4) Assist in OB/GYN procedures

(iii) APRN Pediatrics Privileges

(1)Incision and drainage of subcutaneous abscesses and cysts with supervision

(2) Removal of superficial foreign body

(3) Suturing of minor lacerations, excluding facial, hand, never and tendon with supervision

(4) Circumcision with supervision

(iv) APRN Psychiatric Privileges

(1) Perform psychiatric evaluations including

(a) Psychiatric history

(b) Medica! l history

(c) Background information

(d) Mental statu! s examination

(e) Multiaxial diagnosis according to DSM-IV

(2) Perform written psychodynamic case formulations

(3) Group and individual psychotherapy

(4) Evaluate and order use of seclusion and restraint

(V) APRN - Family Practice Privileges

1. Performs a complete physical exam, obtains complete medical history and records findings. Orders appropriate laboratory and diagnostic procedures. Interprets and integrates data to determine preliminary diagnosis and therapeutic plan.

2. Administers medications and injections in compliance with state law and clinic guidelines. Sutures minor lacerations.

3. Instructs patient / family regarding medications and treatments. Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.

4. Collaborates with physicians in managing acute and long-term medical needs of patients. Provides monitoring and! continuity of care between physician visits. Maintains and reviews patient records, charts and other pertinent information. Posts tests and examination results. Triages patient telephone calls and provides consultation.

5. Maintains patient confidentiality.

6. Attends / participates in department meetings / inservices, Mary Lanning Memorial Hospital quality improvement programs, etc.

7. Performs other duties as assigned.

" Has access to the locked doors on the 7th floor as well as normal hospital access (as assigned).

" Responsible for proper use and operation of supplies and equipment. Improper care or use of resources could result in damage, loss, and added expense to the hospital.

" Complies with department safety policies and procedures. Responsible to help ensure that the safety and! physical well being of patients, visitors, physicians, and employees i! s maintained.

" Needs to be aware of the patient's safety needs and act appropriately on their behalf.

EXPOSURE TO HAZARDS: Normal hospital environment with little discomfort because of noise, temperature, dust, or the like. Occasionally may be exposed to communicable diseases. Occasionally may be exposed to body fluids and agitated patients.

Ability to read, analyze, and interpret medical / clinical chart information, professional journals, technical procedures, or government regulations. Ability to interact therapeutically and exchange accurate, appropriate and timely information with patients, families and other health care professionals. Ability to write reports and chart medical information and understand policy / procedure manuals. Ability to effectively present information and respond to questions from groups of managers, employees, physicians, p! atients and the general public.

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals and to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, decimals, percentages, ratios, and proportions to practical situations; and to draw and interpret various kinds of complex graphs.

ESSENTIAL TECHNICAL / MOTOR: Must be able to utilize and complete required psychological tests. Must be able to use computers and computer programs as required.
